I authorize Go Green Plumbing, Heating, Air and Electrical to charge my credit / debit or bank account in the amount of $0.00 per month plus applicable sales taxes.

I understand that this authorization will remain in effect until I cancel it in writing and agree to notify Go Green Plumbing, Heating, Air and Electrical in writing, or by phone, of any changes in my account information or termination of this authorization at least 30 days prior to the next billing date/renewal date to prevent automatic rollover. I agree that cancellation prior to 12 months of service could incur a penalty charge of $175 dollars. If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day. I certify that I am an authorized user of this credit/debit or bank account, and will not dispute the scheduled charges with my bank or credit card company, so long as the transactions correspond to the terms indicated in this authorization form. Cancellation of the membership before 12 months voids all applied discounted amounts from the previous months’ discounted amounts, that then become due in full to Go Green Plumbing, Heating, Air and Electrical.
